Physical Signature Required at Event

This page is provided for review purposes only. All clients receiving tattoos at the Tattoo Show Invitational must sign a physical waiver at the event registration desk before their session begins. No tattoo service will be performed without a signed waiver on file. You must be 18 years of age or older and present a valid government-issued photo ID.

1. Identification & Age Requirement

I confirm that I am 18 years of age or older. I understand that tattooing is a permanent body modification and that the Tattoo Show Invitational does not permit tattooing of minors under any circumstances, regardless of parental or guardian consent.

I agree to present a valid, government-issued photo ID at the event registration desk prior to my tattoo session.

2. Informed Consent — Risks of Tattooing

I understand and acknowledge the following risks associated with receiving a tattoo:

  • Tattooing involves needles penetrating the skin and carries inherent risks including, but not limited to, infection, allergic reaction, scarring, and keloid formation.
  • Certain medical conditions, medications, and skin conditions may increase these risks. I confirm that I have disclosed any relevant medical history to my assigned artist.
  • Tattoos are permanent. Color fading, line migration, and changes in appearance over time are normal and expected.
  • Results may vary based on skin type, placement, and individual healing response.
  • I am not under the influence of alcohol or controlled substances at the time of my session.

3. Health Disclosure

I confirm that I do not have any of the following conditions that would contraindicate tattooing without prior medical clearance: active skin infection or rash at the tattoo site, bleeding disorders or use of blood thinners, compromised immune system, pregnancy, or any other condition that my physician has advised makes tattooing inadvisable.

I agree to disclose any known allergies, including allergies to latex, metals, or topical anesthetics, to my assigned artist before the session begins.

4. Competition Context

I understand that the Tattoo Show Invitational is a competitive event and that my assigned artist is competing for prizes and recognition. I understand that the artist has been assigned to me through the competition matching process and that I have reviewed and approved the proposed tattoo design prior to the session.

I understand that competition judges may observe, photograph, and evaluate the tattoo being applied to my body as part of the scoring process. I consent to this observation and documentation.

5. Photography & Media Release

I grant the Tattoo Show Invitational and its authorized media partners the right to photograph and video record my tattoo session and the completed tattoo for use in event coverage, promotional materials, social media, and archival documentation. I waive any right to compensation or approval of such use.

If I do not consent to photography, I must notify event staff at registration. Limited photography-free accommodations may be available on a first-come, first-served basis.

6. Aftercare Responsibility

I understand that proper aftercare is essential to the healing and longevity of my tattoo. I agree to follow all aftercare instructions provided by my artist. I acknowledge that failure to follow aftercare instructions may result in infection, fading, or other complications for which the Tattoo Show Invitational and the competing artist bear no responsibility.

7. Release of Liability

In consideration of being permitted to receive a tattoo at the Tattoo Show Invitational, I hereby release, waive, and discharge the Tattoo Show Invitational, its organizers, officers, staff, volunteers, judges, and competing artists from any and all claims, demands, or causes of action arising from or related to my participation as a client, including any injury, illness, allergic reaction, or dissatisfaction with the tattoo result.

This release does not apply to claims arising from gross negligence or willful misconduct.

8. Governing Law

This waiver is governed by the laws of the State of Oregon. Any disputes arising from this waiver shall be resolved in the courts of Curry County, Oregon.
